The MMTCFL website says they "request that you bring supporting documentation with your diagnosis or prior treatment to the clinic at the time of your initial appointment." What they DON'T say is that this is REQUIRED, and that it must be SIGNED by your doctor. Not a PA or a NP, a medical doctor. I brought my entire medical record, printed from my provider's patient portal. Very unprofessional. I made an appointment for Monday afternoon, and 1 hour before it I was telephoned and offered an appointment earlier in the day or the next day as the Doctor "had an emergency" (if it was a true emergency then why could be see me earlier...). When I arrived the next day I was told the Doctor was not in for the entire day, and that I would have to return the following day. They have wasted my time and therefore I will not return.
