5/21 Examination / Crown Preparation / Extraction and Restoration Filling discussed Extraction and Filling will be done at same time Crown is Installed. Treatment Plan lists all work by Zakari Taylor. Never saw Dr. Taylor again 6/4 Message on my phone – Crown came in – Need to make appointment. 6/8 Two different staff mentioned that I would be called when crown came in . . .
