Get directionsMy mom, Susanna Roman, was recently admitted to Sequoyah Pointe SNF. She was there from August 1st, 2025 to September 2nd, 2025. While she was there a short time the care she received was exemplary! I live in California and everyone was so amazing with letting me know if there were changes and how she was. My mom was in isolation while recovering from C.Diff. She told me all the nurses were nice & patient with her :) When she came out of isolation, I called her and discovered her room didnt have a phone but multiple nurse... read more
Frozen pizza & canned green beans for dinner? Seriously? Do NOT send your family members here. It is DEPLORABLE. Couldnt get anyone to come help my mother to the restroom. 5 times in one night, FAIL. Then leave her off her oxygen for 5 HOURS. This place is a hell hole & how sad its employee owned, yet they clearly don't care about their residents. Should be shut down and lose medicare funding. Thankfully, we are recovering at a different facility.

The nurses and staff are very attentive to the needs of the patients and guests. My grandmother fell and had to have a short stay for rehabilitation needs. This is only one of two choices for in care rehab in Tahlequah . It was not our first choice but everything turned out very well. Everyone is very kind and well mannered. The physical therapy unit is great and they go above and beyond to get people back up and going!
Jan 15th 2022 Covid positive employees were aloud to remain working. The patients had to be locked down due to employee error. They hire motivated people that get shut out by the long term burnt out employees. Food is often behind by even hours and some patients may get overlooked and given a sandwich.
