Transitions Hospice was absolutely phenomenal. From the beginning to the end was the most personal, caring support we could have ever hoped for - this was our third and BEST Hospice experience as our mom had graduated Hospice twice before (from different Hospice providers). The whole team were very passionate to me and my family, from the nurses and the equipment guy, and was on top of supplies and medicine we needed. Only complaint is that the all the different nurses took up almost all my time with my husband in his last few days, as I would of liked to spend most my time with him while he was coherent enough to talk to me, from being here at the house or in the phone.
Your team was not only professional it was also very compassionate and understanding during my daughters passing. I was hesitant at first but with all your help you made a terrible situation bearable. I can’t thank everyone at Transitions enough for the care you gave Wendy and support to the family.
Did not have a good experience with Transitions Care with my husband they did not follow through as I was told they would. They were supposed to call me when my husband Toms vitals were in critical numbers and they never did. I wasn’t there when he passed and they never called me after he passed I found out when I got to his room I sent a letter after Toms death expressing the lack of respect for Tom and myself I never heard back or got a response to my letter.
