We cannot say enough about the incredible care Transitions Care out of Champaign, IL provided during our grandma’s passing. Transitions provided excellent care and support to my Aunt a year ago. Their caring words and promptness in setting everything's up was outstanding. We met with Kirk and had excellent nurses and CNAs . It was like a transition . Our family is very pleased and would recommend their services to all. Thank you Team Transitions ❤️
Transitions was amazing and incredibly supportive in our last 2 months with our wonderful Dad. Tom was his nurse and Kevin was his CNA. We could not have kept our Dad home, where he wanted to be, without all of the guidance and help. They are truly angels. Anyone who gets either of them- or both like we luckily did, will be forever grateful! We can't say thanks enough from me and my 2 sisters.
We started with another similar sounding name of a company who talked a good game, but failed miserably when it came to actual service. After terminating the other company and getting Transitions, it was a breath of fresh air. They provided quick and wonderful service and brought comfort to the family member in need. We will be forever grateful to Transitions for the wonderful equipment and care that they provided during a stressful time.
The care my Mom received from Transitions was unbelievable. From my initial call to Jennifer on Friday, April 29, 2022 to my Mom passing on Wednesday, May 4, 2022. Everyone involved Jennifer, Tiffany, Gweena, Karen, Cathy, Amy, Joe and most of all Dawn treated my Mom with the best of care. I didn’t think my Mom was leaving us this quick.
