I want to recognize the exceptional Wound Care Team at Citrus Heights Post Acute--especially their Wound Care Lead. From day one, it was clear that she is highly knowledgeable, organized, and focused on one thing: healing. She is very straightforward and thorough in her approach, making sure every detail of care is handled properly and efficiently. Her communication is clear, and she ensures that everyone involved understands the plan of care and follows through. You can tell she leads by example and expects the best for ev... read more
My grandfather was at Citrus Heights Post Acute and since has made several statements that he would go back if medically needed. Great therapy staff, they worked hard to get him safe enough to return home both cognitively and physically, also recommended equipment needs to be successful at home. Overall great experience and would recommend to anyone I know needing rehab or long term services.
Horrible place to put a family member. My father was there two days and the food was terrible his roomate had debri spread all over the room there were stains under my father's bed . My brother found my dad with his catheter pulled out and feces everywhere and no nurse around. Honestly i don't know how this is able to stay in business
Place is a nightmare, a storage unit for future cripples. "Rehab" consisted of one hour a day, 5 days week. The rest of the time is spent being bullied and ignored by staff. Four hours to be cleaned after bedbound toilet. Call lights might as well be unplugged since they accomplish nothing. I don't care if they're understaffed or what there's no excuse when you are responsible for people's lives.
We had high expectations for Citrus Heights Post Acute after taking a tour as we needed a rehab facility for a family member who'd had a stroke. He spent over a month at CHPA, which is where he passed away. During his stay, on our daily visits he was frequently found unclothed, in need of medication due to agitation, in a soiled adult diaper, or bleeding from an unattended wound. Politely asking anyone to assist resulted in apathy and the impression that CNAs and RNs couldn't be bothered. We absolutely understand what a ... read more
Today, October 12, 2025, my mother was admitted to the ER after poor care at Citrus Heights Post Acute Hospital. My mother was transferred from the hospital to Citrus Heights Post Acute to recuperate in a positive environment, but because the Citrus Heights Post Acute room was stuffy and the air conditioning was not working, she began to feel ill. For two hours it was impossible to attract the attention of the medical staff, although they were on duty and talked to each other, but did not pay attention to the patients. ... read more
Absolutely not worth the 4 star rating on here. No communication from staff, no real rehabilitation for patients, inaccurate records and intake that was not fixed despite multiple attempts. loved one actually came home in worse condition than when they arrived at CHPA unstable and with giant bedsores. Staff were so in hurry to get my loved one out that they miscommunicated the health status, and we had to call an ambulance within an hour of getting them home. When we contacted them the next morning, less than 24 hours after... read more
Do not let your family go here. It is very dirty, the nurses would not respond to alerts and it was very hard to understand due to language barriers. Not once did we see a mop for a week. The food was all blended across the board. They allowed patients to roam freely into other patient rooms. Moms roommate contracted Covid and u til we demanded her to be moved they did nothing. Horrible bed side manners. One nurse even asked my mom "my god have you seen your wound..wow". Scared her. Dr came in and started screaming at my mom... read more
