I am 3 1/2 hours away from David Connelly. I am the medical POA. I am getting very little information (or nothing at all) about the outcome of his visits. I am a bit annoyed that the information is not provided by the provider or the assistant after a visit has finished. It would be great if you could fix that.
Our father lives in an assisted living facility and receives care by a representative of DMHC. This is his fourth month with this program. So far all is going well. Trying to set up the communications system between the medical NP and the family has been a little frustrating but otherwise things seem to be okay. The in house service is a big improvement over what we were doing in the past.
We have been pleased with the service provided by your personnel especially Jeff Lafuria, PA. He does a thorough job, and has the respect of our family.
