Very simple and complete instructions to obtain the readers card are documented on LoC website. The only confusing aspect was actually navigating to the reading room entrance. My daughter and I had a great experience, it was one of her bucket list items that she was able to fulfill this year now that she is 16. They recommend completing the pre-registration form on website, however the day we went it was just as easy for me to complete the form onsite.
