I called and had a talk with Midwest Disability and they sent me some forms to fill out. I sent the forms back alone with copies of the forms Social Security sent meto fill out. I called Midwest Disability 2 times with questions and it took about 2 months and I received a letter from Social Security Disability letting me know that I was approved. I think by me telling Social
