WARNING: They will play insurance games, telling you that your surgery will be covered, then when the insurance company doesn't go for it, you get stuck with the full cost. I was told my extraction would be a $390 copay, and that it would be processed as though the doctor was in-network, even though he wasn't. So now they're trying to stick me with the remaining $1100+
