Went here on to 2/3/23 for a DOT physical everything was going fine until the time for the paperwork they would not provide with the long form with all the findings. They did, however fax to my company the long form, but did not finish filling it out after 7 days trying to get them to fill out the paperwork. The paperwork still not filled out correctly. At this time I am forced to go to another physician to get a DOT physical to be able to continue driving. I will not ever return to this place.
