We are saddened to have to let transitions go for the care of my mother . However , after much pleading , calling for help , after a serious fall Dr.Agusti made some remarks via phone that were unacceptable and not appropriate. My mother , whom is the patient , couldn’t move , hit her head , bleeding and had a possible broken collar bone . His negligent attitude over the phone to us as we called for emergency help were inappropriate to say the least. We had to find care elsewhere right away. Beware of entering into Transitions Care. We do not want others to experience what we did with the company in our time of grief. Please ask for other hospice services references. Our dad entered Transitions Care through their Ft Wayne, IN office in June. Our experience was anything but favorable. A broken bed was delivered to their house before he was transferred from the hospital. The delivery person said that it would be replaced by the time he was home. They weren't there when needed. Didn't look at my moms bed sores ....couldn't tell us that she was close to dying..... always seemed to be in a hurry. I don't recommend them. Sorry.
I don't know about the CNA's or nurses but the Administration staff is terrible. The President and Vice President lie and they don't pay their bills. They don't return calls or emails.
