RecommendedOur grandfather went into the hospital on Friday evening. We were given a grim prognosis, we knew it would be fast. We knew we did not want him to suffer. RecommendedWhat a blessing all of these people are! They were with us every step of the way and led my father through a very peaceful transition!! Don't let the town they are listed in (Huntley) scare you! We live in Joliet and it never stopped them from being here when needed! Mos of the team of caregivers we had for my Dad were actually from around here. I can't say enough good about them!! If you need a Hospice Group, please consider using Transitions! A decision I don't think you would regret!!
RecommendedToday I met with Mr. Palazzo and I was very impressed with Transition's philosophy and comandments. I truly felt that the patient and families are a priority with their needs being personally met with human contact, not just a phone call. They put personal care back into Healthcare. Care plans are developed for each individual patient and family, recognizing every ones unique needs.

We are saddened to have to let transitions go for the care of my mother . However , after much pleading , calling for help , after a serious fall Dr.Agusti made some remarks via phone that were unacceptable and not appropriate. My mother , whom is the patient , couldn’t move , hit her head , bleeding and had a possible broken collar bone . His negligent attitude over the phone to us as we called for emergency help were inappropriate to say the least. We had to find care elsewhere right away. Beware of entering into Transitions Care. We do not want others to experience what we did with the company in our time of grief. Please ask for other hospice services references. Our dad entered Transitions Care through their Ft Wayne, IN office in June. Our experience was anything but favorable. A broken bed was delivered to their house before he was transferred from the hospital. The delivery person said that it would be replaced by the time he was home.
