Get directionsThis place looks gloomy, depressing, filthy and smells like everyone has peed themselves for a week with out a bath. You can instantaneously see they are very short staffed. I bet they have plenty of staff when the state comes so they can stay open. Poor patients, poor nurses, poor CNAs.

It is unfortunate that this was your impression. Since taking over as Administrator in May 2019, I am pleased to report that our overall Medicare "5 Star" rating has improved from 3 stars to 4 Stars, and our Quality Measures rating has improved to 5 stars. I am committed to meeting the needs of our residents, families, and staff, and even include my cell phone # on my business card. Rob Zitsman Administrator
Worst place to take a love one. seems like they care less about their patients. You'll feel lost in the system here.
Unfortunately i have placed my trust in a very unprofessional staff and facility. In a difficult time my family was going through when our uncle was very ill and weak one of there so called "nurses" was supposed to be assisting him to the rest room he was a bit slow to move like many of the other patients , the" nurse " pushed and hurried him in a very impatient and rude way almost causing him to fall and making him feel unsafe . After he asked her to be more considerate (in spanish) she said to him "english ...SPEAK ENGLIS... read more

Much has changed at Escondido Post Acute Rehab since 2016. We have a new leadership team that is focused on meeting the needs and expectations of our residents, families, physicians, and our staff. Families and prospective patients are invited to stop by for a tour and to meet me and other members of our team. Rob Z., Administrator
If I could give this place a ZERO (0) I would. The 1 is only because of a few of the people that work there, that helped me after the fiasco with my mom! My mom had knee replacement surgery and was transferred there after her recovery at the hospital at the beginning of January 2016. From the moment my mom & I got there we were COMPLETELY ignored by the Head Nurse...who told me a few times that she was "overwhelmed" and she would get to us when she could...WHAT??? I personally witnessed this head nurse ignore other patient... read more
My grandmother is currently a patient at this facility. I must say, the standards are as low as any other skilled nursing facility she has been admitted to. There is a lack of nursing management. In fact, never see a nurse in charge follow-up on the CNA's after diaper changes or feedings. My grandmother has a pressure sore that has not been treated. In fact, the patient next to her does not get re-positioned every 2/hrs. We were their for 4/hrs and the poor patient was still on her back. We mentioned it to CNA that the p... read more
I wouldn't give this place any stars. This place killed my father. The doctor prescribed a fentenel patch for a 87 year old man on oxygen. I went in to find him not even hooked up to his oxygen. He was there two and half days.
My mother and I had an awful experience there while picking up my grandfathers belongings. After he had passed away we drove 9 hours to the facility so that we could gather his belongings. When we arrived at the facility 24 hours after he passed away we had discovered that they put all of his things in trash bags and put those bags in an office. We specifically told the facility that we wanted to pack his belongings. As you can imagine finding his things in this way added to an already difficult time. All the facility could ... read more
