This facility is hard to review as the nursing and therapy would be a 4 and 5. We especially felt Dan, the therapist was exceptIonal and communicated each week. That being said, the communication from here is terrible. It is hard to get in contact with anyone on the administrative side. The case manager, Ms. Scott , never called back until after multiple calls over multiple days. She didn't even visit my Mother when she was there, yet was doing her care coordination and discharge. Don't let the ratings on medicare and other review sites fool you. I researched extensively before sending my loved one to this SNF. I was relieved and grateful when my loved one was accepted. However, that relief soon diminished. If your loved one requires multi levels of therapy and Rx distribution, I would NOT recommend this place. The facility is beautiful and clean, food was good, admissions and the receptionists are great, and the physical therapists are good. I can not believe that this facility is able to get rid of patients with dificult diagnosis and treatments, instead of helping the families that trusted them with the care of their family member. If wrong doings, or mistreatments are reported by a family member, patients, are subjected to Managements retaliation, and eviction when all along the answer is to properly care for the patient.

This facility was highly recommended to us, but unfortunately, did not live up to the hype. There were some very exceptional employees, but overall, the attitude there is poor. Administration is hard to contact and does not return phone calls. We were left waiting for two hours for a meeting at one point to discuss how my mother was doing and when she was scheduled to come home. I was told this facility is great but it's not. The staff in the evenings and weekends are careless. They don't clean the rooms. They leave trash, spilled items, feces, and body fluids on the floor. I got into the habit of checking my mom's bed after I once found that their staff had made the bed but left urine soaked bedding underneath. They were going have her sleep in that! One of the CNAs told my mom to just defecate on herself and they would just come change her later. My mom isn't bedridden. She just needed a little he... read more

My mother was admitted to North Houston Transitional Care on February 4, 2019. When she arrived at the facility her blood pressure medications were not available. I was informed that the medications would be delivered to the facility before midnight. After arriving at the facility the next morning I was informed that my mother did not receive her meds because the pharmacy failed to deliver them. They knew that the physicians at the hospital had a difficult time regulating her blood pressure. It was very important that sh... read more

I would not recommend this place. My 79-year-old Dad was transferred here for skilled nursing, rehab, and wound care. The rooms are nice as well as the facility. My family and I take turns staying with my Dad because we didn't want to leave him alone. He has a large wound on his heel that has kept him off his feet so his body was deconditioned. He also was getting over pneumonia. He wasn't assessed until a couple of hours after we got there. The staff didn't come around much and when they did they had a scowl on their f... read more
