I came to the clinic for one reason: to be drug tested for a new job. I felt like the forms I had to fill out required way too much information or why I was there. Emergency contact, pharmacy preference, personal physician, etc. was way too much info needed just to come in for a drug screen. My future employer instructed me to go to Fast Pace for a drug screen and it was not my intention to become a new patient there, so it was frustrating to fill out all the mandatory information that wasn't really needed.
