Since switching doctors, my prescriptions are not renewed after my visit. Anthony would renew all my prescriptions at my 3 month follow up. My pharmacy had the updated authorization. With Dennis, the pharmacy is always needing to call and get approval. I have asked for all my prescriptions to be renewed during my visit, not as needed. It delays my meds

Primary care availability? Drs. Cendejas and Morris no longer available? Has the $1.5 million dollar settlement been settled/decided since personal client/patient information was disclosed/revealed? Applied for a position with Revivals PS and no interview was offered/provided/allowed. Have always been supportive as a supporter, volunteer and donator. Thank you!
Your female receptionist (the one with dreadlocks) from the “blue room” clinic was very rude. I took my husband & my elderly, disabled dog with us, who was by the way, inside a backpack carrier the whole time, she has separation issues & when I was in the clinic for my physical exams, she was softly whining looking for me & to comfort her, my husband softly whistled a song to her to help calm her & your female receptionist snapped at my husband & said in a stern voice, verbatim: “stop your whistling!”. As long as you never request/need refills, then DAP Sunrise is the place for you. Contacting Kinji Hawthorne MD or his Asst on a Friday morning, practically guarantees you no response. With the pharmacy contacting them and I, they simply refuse to respond to ‘My Chart’ requests along with direct messages/voicemails. How hard is it really to pick up a phone & respond to a pharmacy? 3 minutes? Walgreens is happy to let me know what vaccines I’ve had or need, but my GP doesn’t give a flying about my med refills. Truly not loving this new system. I’ve been assigned a new Dr/PA? Not really sure of his actual accreditation. Only met in person once and since have been reduced to phone appointments and now not even a phone call but my chart appointment. No idea what that is about. How can I be confident in my health care if I can’t even get a Dr appointment with a Dr or an actual appointment with a real person?
I had an appointment by phone that was cancelled 4 minutes before the appointment time. When I called at the appointment time, I was told I needed to reschedule because I was late. The provider is suppose to call me for my telemedicine appointment.
I haven’t been to DAP in over a year. I’ve been waiting to be assigned a PA or Dr. I’d would really appreciate it I could be seen at DAP as soon as you can. Thank you. Mark
