My sister is in need of skilled nursing for a couple of weeks and upon arrival at this place, was told that she would need to be in isolation for 14 days. In that "isolation" she was placed with another patient who turned out to be VERY CONTAGIOUS with Candida. My sister is being told that she will have to "isolate" (how was it isolation when she got a disease form someone else?) for another two to three weeks. Upon being told how detrimental this all was to my sister's mental health, I was told "you don't understand" in ... read more

Las Colinas Post Acute 800 E 5th St, Ontario, CA 91764 Rehabilitation center My mother was admitted to receive speech therapy and physical therapy after suffering 2 strokes. However after 14 days she developed a bed sore. I never received confirmation that she even began receiving therapy. I never heard from a doctor or case manager to discuss her treatment. Every time I called to speak with someone I was told to call back or that they would call me back and no one ever did. When I called to talk to my mom, she would be asl... read more

I wouldn't leave my worst enemy there. They DO NOT CARE about the patients. They never give them water and they completely ignore them. The business office/Admin team, completely useless.

This review more-so revolves around my experience and observations of the hostile and unprofessional behavior exhibited from the "receptionist" Brianna (as she so sarcastically provided me with when I inquired). I came here with my aunt to pick up my grandmother who was being discharged this evening (as I was the transportation). I, along with my aunt and another woman, was escorted inside by a very nice gentleman so we can check-in and wait to speak with someone for assistance (obviously being mindful of social distancing... read more

This is a convalescent home of horrors and every elderly person's worst nightmare. The motto seems to be check in and check out because no one has an interest in the patients or their concerned family members. They almost broke my mom. They drugged her and kept her from being able to talk to us. Someone reported them to the ombudsman while my mother was there. When I called to request they send my mother to the hospital, after weeks of them giving me and my family the run around, they hurled accusations at me! Excuse m... read more

Run away from this place. The lady at the sign in desk basically didn't want to assist anyone walking in. Then the ladies at the desk past the sign in desk pretend like they don't see you. When you walk in you automatically smell a mixture of feces, vomit and just sickness. My father was here "short term" and within 4 days of their "care", my father got sepsis. Had to be admitted into the ICU and fight for his life. When I contacted to speak to the nurses they informed me that they neglected to even take his vitals during h... read more

My sister was there for several weeks. Terrible place. My sister was blind, had palsy, RH had a stroke. They were told she couldn't feed herself because she shakes. Most of the time they set her food on tray in front of her. They took her 2 beautiful blankets she brought with her. They are unfit to care for a person that need help. CNAs and Nurses didn't answer her calls for help. I will hope everyone of them ends up needing care someday and get the same type of care they gave my sister. I will curse them until the day I ... read more

The only reason there is one star is because I needed one to post the review. My father has been admitted here twice and both times were not very pleasant. The last time he stayed here he had a wound that needed to be dressed daily and because he needs dialysis he gets nauseous very often. When I visited almost every day, they told me his wound was healing fine; when in fact his skin was "healing" but very red and irritated and swallowing the stitches. Twice he was "found" on the floor in his room. They put him on antidepres... read more

If I could take away stars, I would take a million away! STAY AWAY from this place. They will file false allegations against the family stating "financial abuse", they told my aunt that we just left her there, that we didn't want any contact from her. Then they tell us that she doesn't want any contact with us. They are billing insurance companies for services that are not preformed, they are literally holding her hostage, my poor aunt was told "you are not allowed to communicate with any of your family, we have already told... read more
