Dr Yamashiro is the best. My dental health is so much better since I’ve been under his care. There is WAY TOO MUCH information required when I check in for a visit. The form is long and requires me to write in so much information that you already have. I’ve been a client for 20+ years and a patient for more than 10. You should already know my name, age and lots more about me instead of requiring that tedious form.
