I want to express my enormous gratitude to nurse Vicki Campbell who is instrumental for my father's care in Pleasant Hill Post Accute where he resides since 2023 . Vicki goes way beyond all expectations and delivers not only care for patients but also peace of mind for the family . Her professionalism , kindness , quick thinking and constant willingness to help made a huge difference for my father and all of us !

I want to express my gratitude for the excellent care at Pleasant Hill Post-Acute Care. I was admitted with a fractured pelvis and could not walk. Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! The facility had the best care for my mother. The facility and the whole team & staff including all the nurses were very kind. I want to say a special thank you to Zymond, Marilyn knight social worker and Elaine for helping me transition to home care for my mother. It was very comforting going forward. Pleasant Hill Post Acute was an amazing rehabilitation program, and I'm so glad I chose it. I really appreciate everything they did to help me recover and regain my mobility after surgery. I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. My husband spent 10 days at Post Acute and was very impressed with the care he received. The CNA's were very attentive and pleasant. They treated him with dignity. He liked all the physical therapists. I found tfw staff very helpful. He said the food was ok but he was on a soft diet. Highly recommended.

This facility has an amazing staff. The nurses like Martha and CNAs  - Olivia, Dalia - were always on top of everything and truly attentive to my needs. The therapists - Jordan, Sara, i cannot remember their names but - were highly motivated and consistently encouraged me to stay active and keep moving, which made a big difference in my recovery. Special thanks to Bianca, the case manager, for making my transition back home such a smooth and stress-free process. I'm very grateful for the care and support I received here.

This was our third time dealing with PHPA! Back in 2020 during Covid my mother in law ended up there and then again in 2022. Both times she received excellent care and great pt and ot. The staff is very professional and care deeply about their patient care. Recently my 92 year old mother fell at home and after having a partial hip replacement was able to get into PHPA. The staff at Pleasant Hill Post Acute is very friendly, attentive, and cheerful. The therapists are excellent, the nurses and CNAs are efficient and empathetic, and the front desk staff is welcoming and helpful. Nikki, Mariam, and Olivia deserve special thanks for their great attitude and support.
