I broke my hip on Dec 15th and operated on Dec 16th so was in the hospital for a week. They recommended Pleasant Hill for rehab. I couldn't have had better care. The physical therapy was excellent! All the nurses, doctors, CNA's and PT's were excellent! Highly recommend this facility to anyone who needs it! Also I am a dialysis patient, they were great on my dialysis days giving me snacks to take with me on those days.

This has been (very) long overdue. My uncle stayed in this place for weeks and went through a lengthy process of rehab and was finally able to come home. I'm actually typing this review on behalf of my dear aunt. She would like to thank everyone who took care of my uncle, but most especially Marj, whose truly a blessing. She would rave at how much Marj helped her a lot during my unc's stay. Marj was always pleasant, cheerful, helpful, and most specially has a lot of patience. My Dad was admitted at Pleasant Hill Post Acute for almost 2 months and he was treated with genuine care and compassion. The staffs like Alex LVN, Rey CNA, Mavic LVN Case Manager, Alvin or Marvin CNA, Julie CNA, the speech therapist Asian Lady, Alexis RN, were the ones that helped him so well, can't thank them enough. This facility is highly recommended. Thank you guys so much for being my dad's family during his stay there.

I had a teriffic encounter with their LVN, Kate F., this morning. She was extremely sweet and helpful while offering her assistance with one of their residents and his family member. The facility is still closed to family, and outside visitors and guests, but she facilitated a meeting with the patient from inside the building, while we were outside his closed window. The parking lot is extremely small, with lots of no parking signs. Note: their sign says Oak Park Post-acute, rather than Pleasant hill post-acute.

My 97 year old mother was admitted for a 3 week stay. Throughout her entire stay she was treated with love, respect and great care. All the nurses and staff were extremely attentive to her needs and always greeted her with a smile and warmth. She also said the food was very tasty but was a bit on the 'soft' side. We used PHPA as a rehabilitation provider recently, when an out of town family member had a fall while visiting and needed acute rehabilitation placement. PHPA was incredibly accommodating, professional, polite, patient, flexible, and comforting in their entire approach. The facility is lovely- clean, lots of indoor and outdoor gathering options, good food, activities, access to professional and support staff, conveniently located. I have recently been a 'guest' in this facility, after breaking my hip, and all my fears about being in such an establishment were immediately put to rest. The management and all of the staff do everything possible to make you feel comfortable and well taken care of. The food was extremely well prepared and everyone is given an opportunity to discuss preferred food dishes and dislikes.
