Monica Blanchette my alcohol and substance abuse counselor didn't fill out my paperwork according to DMV standards . She submitted her own paperwork with no beginning dates of treatment and no ending date. And no dates in between. I did 15 secessions with her and submitted all clean urines and DMV operator control rejected her paperwork for me to restore my license.
