I made an appointment with Dr. Berkowitz on Oct 29th for November 22. As you can see the email went to spam. I received a follow-up to the 22nd telling me yesterday that my intake paperwork needed to be in with in 48 hours of appointment. I have had both hips replaced since the end of March and have been on pain meds since December. I wear a pretty serious back brace as well as a brace on my knee and neck.
