I would rate as 0 stars if able. My father nearly died during his two-week stay. When he was transferred to this facility, no one knew which room he was assigned to-or even that he had been admitted. The next day, he was still wearing the coat and shoes he arrived in. His food trays were left untouched, he was not given water, and days went by without lab work being done. If you care about you people, omg do not send them here. Especially if they have memory problems and can't advocate for themselves. They will wait 30 to 45 minutes minimum if they have to use the toilet. By then it's too late and they soiled themselves and are shamed to death. They won't get their physical therapy regularly or when they do it's not useful and doesn't help. People are left to sit until their 100 days of Medicare are used up with no actual rehabilitation going on. If a person can't feed themselves, they don't... read more

Do NOT send loved ones here if they have any memory issues or cognition issues. Profits over care. Patient abuse daily and horrible governance. They expect patient to be able to ask for help rather than doing what doctor orders regarding proactively care. Expect suffering and mistreatment of your loved one.

Stay away! Staff not helpful or friendly, dirty place, meds not given on time, the inmates are running the show, no supervision.

Wish I could leave 0 stars. I had read the reviews on this place after my grandpa had been admitted. Like the other reviews have said do not put anyone you love with declining health in this facility!!! They have zero urgency in helping my grandpa. He needed to go to the hospital due to a returning infection and they did nothing but wait around and put blame on the head nurse for not doing anything. So to start off if I would not ever give this place the 1 star, it is a horrible facility, if you want to give your loved ones a chance please please send them elsewhere. My family member that was in there care was not cared for, this said family member had went 20 days without eating !!! 20 days that is so ridiculous!!! My husband stayed here for two months in 2014. His experience was positive. In 2016 when he had to go back in everything had changed. None of the previous employee's were still there, it was a completely new staff. The wages had been cut. We found that out from an ex employee. The place felt depressing and it was obvious that the employee's weren't happy. The food was horrible. My husband was served regular food even though he was diabetic. IF YOU CARE FOR YOUR LOVED ONES, I URGE YOU TO STAY AWAY...!!! Horrible! Awful! Dirty, smelly, and staff ignores patients. My father was there for 5 weeks of rehabilitation, but his function degraded significantly while there! Staff told my father that it was against the law for him to get out of his wheelchair(my father is ambulatory). Staff removed my father's personal property to cover-up a lack of care. Also, he came home with bedsores. IF YOU CARE FOR YOUR LOVED ONES, I URGE YOU TO STAY AWAY...!!!

This place is awful, had a loved one in there, they didn't tend to him what so ever, didn't know what they were doing, and were very rude and careless. One of the nurses gave an IV drip, the IV drip wasn't working for god knows how long and they didn't know how to get it to work again and didn't seem to give two shits and left it that way for awhile, first of all, if you're an RN, you should know this sort of thing..
