I've been to a lot of physicians therapy in this area die to injuries with my back, hamstring tear, and others. However, never once have I been taught how to live my life with certain exercises that I'll be able to do at home until Horizons. Sara was my therapist and she knows exactly 💯% what she's doing and specifically hope to help me. She's extremely knowledgeable, very personable, and truly cares about her patients. I can not say enough about Horizon team and Aftercare. They have been amazing to my mom and me during our process. They all we so kind, supportive and compassionate with everything. Even the day my mom passed. Sharon, Heather and all the other nurses and staff were there every step of our journey, with compassion, empathy and kindness. And Loretta was a jewel. Horizon Health Care & Hospice get 5 stars from me. I had total shoulder replacement surgery one month before my husband’s passing. There was no way I could take care of him any longer one armed. The hospice nurses, Sharon, Heather, Keira, Trinda, and bath aide Ozry were exceptional. The Social Worker Rick was so good at finding a home for my husband at Olena’s Care Home. She’s a saint for the care she gives. Thanks so much to all for their compassion & empathy.

Hannah contacted me very soon after I was discharged from open heart surgery. She visited very soon afterward and was my nurse until I was discharged to start St. Al's heart Rehab last week. I appreciate that Horizon was able to schedule the same nurse so there was consistency with communication. Hannah is both a professional and personable young nurse. PT was ok also. My recovery has been so fast that a little coaching from them seemed to be all I need. Thanks, Horizon

Everyone at Horizon did a great job for me. Always scheduled my visits well and worked with my schedule. Provided care for my wounds and made sure I was healing. Would recommend. Thx.
