My mom was taken to an appointment at InnovAge San Bernardino by her care facility, and what happened to her there is absolutely unacceptable. She has Parkinson’s and limited mobility, and when she asked to use the restroom, a nurse escorted her. My mom asked the nurse to hold her phone since she struggles with mobility, and instead of just holding it, the nurse put it in her pocket. Update 3/31/2025 Very disappointed in the care I have or should I say have not received. Considering returning to Kaiser…soon! Excellent home visits with Claudia, Enrollment Representative and Home Health Nurse, Camille. Looking forward to my services starting 2/1/2025. Leaving Kaiser. I’ve been a Kaiser Member since 1969. Dear Innovage, Doctors, social workers and staff I am writing to express my outrage and disappointment regarding the substandard care my mother, received at Innovage PACE of San Bernardino. Despite being enrolled in your program, she underwent a botched eye surgery that has left her with irreversible vision loss. Furthermore, the lack of follow-up care and communication from her healthcare team has been appalling. My mother was seen by the doctor last week for flu like symptoms. doctor prescribed medication . But no orders sent to the med nurse . So my mother has suffered until Monday.
