I recently moved to South Bend, IN two month's ago and needed to establish a new primary care provider (PCP). I was able to be scheduled and seen the soonest with Dr. Jordan Hatch, M.D. at Trinity's Northwest Family Medicine office in South Bend. After having my first visit yesterday, 04/30/2026, I am extremely happy I chose Dr. Jordan Hatch as my new PCP. Dr Hatch is caring, listens to you and solves the problems. I would highly recommend him. The office staff is friendly and is easy to get in for an appointment, and answers their office calls quickly so you are not holding on the phone long periods of time. Our best doctor ever!!!
