The information requested prior to my appt with Van took too long to complete n became frustrating by time completed. This much info should be mailed out so elderly can complete n bring with them to appt as doing so much on iPhone made me anxious. I know new company but all info you requested was already in my patient file. I’m 73 n dealing with things by iPhone etc is more difficult for me.
