I wish I could give this facility zero stars. Any question you ask staff, expect to get attitude and resistance. They will tell you that “protocol” is to rotate patients every 2 hours. I can assure you, if you sit with your family member, you will never witness this happening. When issues were addressed the only response received from management employees was that “staff needs additional training”. With that said, if you don’t mind leaving your family member with unqualified nursing staff, this shouldn’t be an issue for you.... read more

As always, it is a challenge with getting loved ones cleaned. The staff really tried their best and are always really nice and responsive. Between my husband and I we were there three times a day faithfully to ensure my dad was attended to. Unfortunately, I always try to be fair with the rating but finding my father three times in urine soaked diapers in late day is unacceptable.

If I could give it zero stars I would. This place is a nightmare. Actually scary! I was lied to right to my face twice regarding two different situations with my loved one. Need a shower, good luck getting one. When my loved one was lucky enough to get a shower we had to dry him with a bed blanket because they had no towels. I was there every day for my loved one and even with a mask on the hallway that I would need to go down smelled so strong of urine it would burn my eyes and make me want to vomit. I was recently a resident at the subacute at autumn lake.!! I have only the best things to say about this facility. All the staff are extremely devoted and tentative to all the residents needs. The nurse and certified nurses aides took the best care of me. They responded to all my needs in minutes. The rehab team was beyond amazing they really brought me back to my prior level of function. Facility is beyond beautiful reminding me of a 5 star hotel. Would definitely recommend this facility to anyone in need of rehab!!!

I'm glad that I was available during Mom's first stay at the facility. There was an issue about cleanliness in her assigned room. I reported the issue to staff and Mom was moved to a cleaner room and housekeeping was immediately notified. The Directors, managers, doctors, nurses, therapists, health aides and other staff were attentive, kind and polite with Mom, other patients and visitors. I witness the care provided to patients.
