Transitions Hospice has a wonderful, caring staff that goes above and beyond to assist families through their journey. Their knowledge and skillset help to make the patient's support system and family feel confident that they are receiving the best of care. I recently met with Jacob to learn more about hospice and all that it offers. His gentle and kind demeanor about such an unknown, but somewhat uncomfortable subject for those that are not familiar was gentle, kind and so very patient with the explanation. I would highly recommend him to any of my family, friends or clients at anytime!
