Referral here for a scan. Requested new patient paperwork via email (extensive) and completed prior to appointment. This is a must! In order to complete all these forms legibly it will take an average of 30+ minutes. Include a copy of your living will and medical POA. Every time I have been to the office for my pre-op, my initial visit my postop visit there has not been one staff member that has not been amazing and kind and made you feel very welcomed. The doctors there are very knowledgeable and they make you feel like you are the number one priority of the day, I am a nurse and to see how the doctors interacted with myself and others is amazing and I trusted them through my surgery and my aftercare
The entire staff are exceedingly helpful, thoughtful and efficient. They worked hard to make us comfortable and got us in and out quickly. The facility was immaculate and the parking is exceptional. Thank You VIV.
