The doctors office was really good. I called him. Let him know I was running late. The staff were amazing all of them. They work in a synergistic way completing a common goal and that is the patients well being and comfort. Id suggest this institute to anyone needing the services they provide.
Referral here for a scan. Requested new patient paperwork via email (extensive) and completed prior to appointment. This is a must! In order to complete all these forms legibly it will take an average of 30+ minutes. Include a copy of your living will and medical POA. Best to do this at home and you will only need to provide your insurance card(s), drivers license, and stack of completed paperwork when you arrive.
