Get directionsWhen my Grandmother needed Physical therapy we chose for her to go to vineyard. The building is always clean and smells good, and the team was amazing at what they do. Everyone one was attentive in caring for her and the therapy team got her up and moving way more than we ever could. I would absolutely recommend Vineyard if you or a loved one needs rehab!

In general, the responsiveness and sincerity of staff here has been really good -- better than the other SNFs I was in during the past recent months. However, the management needs to expedite communication to doctors -- unless it's on the doctors' end that they are hard to reach. And VERY important for the patients/residents here: Management needs to enforce employees not smelling like cigarettes or heavy perfume while on the premises. Help and courteousness great help all around. Very thankfulwill try to set up songs on my phone. And learn how to get on it. Do expect to listen to a zoom but hope at eleven am not to bother neighbors in room. Happy camper now and yesterdays

My mother-in law is in rehab here and we are so grateful. Because we have aging parents, I'm very familiar with many skilled nursing places in the Bay Area. Vineyard is so much better than most. All of the staff are friendly and caring. They have great activities and a lovely enclosed patio to enjoy the weather. My husband, dogs and I are so appreciative of the Vineyard staff. Thank you!

My dear friend has been a long-time resident at Vineyard Post Acute for almost three years. He has advanced cognitive issues and is bed-bound with fluctuating awareness. The care during the pandemic was extremely challenging with low staffing done primarily through registry temp CNAs and nurses. There were many issues with his care during this time. A best place for post-acute rehabilitation. It is a safe and supportive environment. Staff are compassionate.
My Sister had to do some rehab there from a bad fall, with a broken shoulder and other injuries. They were very accommodating and took very good care of her. Their staff consists of RN's and CNA's, which are professional and very kind and helpful, going the extra mile to care for elderly, sick patients. In a word (minus the food) spectacular. The staff at Vineyard Post Acute took amazing care of my rehabilitating 88 year old father. My dad is not a primary English speaker and is hard of hearing. They did everything to accommodate him and helped him in every way. There are a couple knocks - 1, the food. I get it, they are a rehab hospital not a restaurant, but when your an old guy and incapacitated some of the only joy you look forward to each day is lunch and dinner. Vineyard Post Acute in Petaluma took on 29 evacuees from Santa Rosa Skilled Nursing Facilities who were evacuated from the fires. These residents were triaged to our dining rooms and activities rooms. Our administrator, director of nursing and staff stepped up to care for these patients and residents. Staff from our sister facility assisted in working all shifts to cover the floors.
