Everything was goof as usually, but I did not like the Patient Form I had to fill out for being a patient there for 50 years. Way to many Personal questions for any person to answer. I think you need to reconsider that form like it used to be.

Associates were very efficient. Even though I had two appointments (mammogram and pap), I was finished with both in about an hour. Overall, receptionist, mammogram tech, nurse, and Dr. Julie Jones were stellar.
