I filled out the forms online with a customer services rep on the phone. That was very helpful. The problem I had is the first signature block, I filled out, many times. I keep getting emails from you asking me to sign the form. I tried to respond once but it put me through the full form again so I didn't. The location has a full copy of the executed form. On line forms are a value only if they work.
