My partner was admitted into Sonoma post acute after being in another facility. The care he got Sonoma post acute was great. He was admitted not being able to walk on he's own couldn't have a conversation. He's cognitive was bad. wasn't eating he ended up with the feeding tube. The staff there was excellent. His nurse. Amy was great. His speech therapist Christina actually got him to start eating peanut butter and jelly sandwiches puréed foods. He started being social again. After 4 months in and out of hospitals and skilled nursing facilities, I was admitted to Sonoma Post Acute. The facility is nice and clean (no nursing home smell!) with activities and tasty meals. However it is the employees that make this place special. It is not just a job for them, they truly care about the patients. The PHYSICAL/OCCUPATIONAL THERAPY department is EXCEPTIONAL. Saw the review (by Augusto C) and had to respond giving an unbiased review. Sonoma Post Acute has been wonderful to my mother, and she is thriving there! She has been at other facilities after several falls, so we are familiar with good and bad of healthcare facilities. First of all - this place is clean, and smells clean! A welcoming positive environment. The management team is caring and goes out of their way to ensure that not only the patient is cared for but the family members are as well. The nurses and CNA's are caring and adjust to each patient's request.

This is the best place in Sonoma to receive great quality care. I would recommend it to any of my family members or friends in a heartbeat. The staff truly care about each individual patient and the nursing team is very talented and experienced. The building is beautiful and feels comfortable, which is very unique for these types of facilities.

S0S: the Sonoma Post Acute is the worst nursing home on the face of the earth. Patients in their care are sent to local hospitals emergency room daily due to severe blood infection. They starve, neglect, and abuse patients. Their beds are infested with bedbugs. Their food is very bad, no one eats it. The health department should shut down this facility. Do not leave your family member at this facility. They do not care about the patients. All they care about is making money. The so called nursing assistants are unqualified. They do not give patients enough water or food. Patients fall there and get hurt all the time. They keep them sick on purpose so they can collect from medicare. The doctor shows us just once a month.

This place is the most unprofessional place ever. They are rude to their employees and don't treat them with any respect. They don't appreciate their hardworking employees.I don't understand how this place is still running....

Beware They do not communicate with you about your loved ones, they send them to doctors appointments with someone elses clothes. No return phone calls. Sheets soiled They lie about incidents and do not take care of your loved ones. So sad I can not wait to get my husband of 33 years transferred out of there. Director of nursing will not return calls. A week after being in the facility my husband does not even know who I am. They tell the transport people totally different stories than what they have told me as the w... read more
