Disorganized, rude, unprofessional

Had procedure scheduled for 11/03; arrived in timely manner to complete paperwork. Sat for over 2 1/2 hours before being called to back area. No one introduced themselves; no employee had visible name badge on. Informed would need IV contrast for procedure-which I explicitly refused as allergy to IV contrast. Left sitting while tech talking to Radiologist-never got back to me. I left facility without having procedure. Waiting room is a disaster. Main door does not close. Mini blinds are in shambles. Front door is covered with old dirty tape that has been half removed. Not a professional setting.
