Worst place I have been to drove 3 hrs to get my sons eyes chkd only to be sent home with another kids information which is supposed to be confidential and now can't even get then to fax or mail the correct visit information wouldn't want my kids info in a strangers hands weird how they can't even mail the correct info to the address on his paper work but they'll send u home with another child's info
