On positive note, staff was great. Otherwise, food is the worst I've ever seen. To give PT bread when they are told he cannot eat it. This happened over & over. Canned spinach yuck, zucchini, 4 days in a row. Mashed potatoes, nothing on them, could go on, & on. Found him in bed naked from waist down twice. Not acceptable. Sometimes meds were 1 1/2 hrs late. Know you are understaffed, but really. Would not let me take him to Dr appt because you did not make appt & you would not get your money. Could go on & on but w... read more

Rude nurses and cna's wanted me to stay in my room and I don't feel that's good on anyone to be confined to there room medication was always late even though I was suppose to get itevery 4 hours and one time I didn't even get it had to complain about itthefood was discussion and always bland and always had the same seasons on it that was discussion now I know why I got depressed want to walk out few times but I needed my antibiotics others wise I would of walk out need to learn respect

The nursing aides in the facility were all pleasant, but niceness is not the only requirement of a good skilled nursing facility! There was absolutely no communication between the facility and the patient’s POA regarding his medical care and therapy progress. The POA signed up for the online portal in order to monitor his progress, and there was an error syncing his chart with the portal. This was brought to the attention of the facility and they did nothing to fix it - instead they kept apologizing that it is “a new system”... read more

The employees were very nice and helpful. I never saw a doctor while I was there. The food was less than desirable. Half of the items on the trays were missing even though they were plainly printed on the menu. I care given to other patients was poor, luckily I got some of my strength back so I could do some of the things I needed to do like go to the bathroom on my own.

She came for stroke rehabilitation but not much was done. Physical therapy worked with her walking a little but not much was done on regaining her speech. She was stuck in her room most of the day with little contact. Her verbal communication actually got worse after admission. Also, she was given pureed meals which was not necessary. If you were afraid that she would choke, you should have had her eat in the company of someone else. How is a person supposed to eat a pureed turkey and cheese sandwich, I mean, yuck! E... read more

We got conflicting information several times over the month that my mother was at Avalon. Except for a couple shining stars, aides seemed less than interested in tending to the patients. There was a man sitting in a wheelchair and when I passed, he asked me to take him to his room. As I don’t work there, I could not do that so I told the aide. . Over the next half hour he repeatedly asked me to please take him to his room as he had to use the restroom. I told the nurse and she said, “He’ll be OK. He can hold it.” I really d... read more

Dr Prey ordered Oxcycontin upon admission for pain of hip replacement - nurses did not believe me that Opiods haveNO effect on me for pain relief… so for 3 days after recent hip surgery they continued to give me OxyContin before switching. Dr Prey also could not find time for me despite 4 requests to speak with her. Your PT & OT staff were very helpful as were most of the NA’s. The aid did an excellent job, speech therapist did not do what she was supposed to do, and there were 2 nurses that did not provide services they were supposed to.
