Get directionsThe staff excluding the social worker was good.We were told that we would be apart of the evaluation meeting and that never happened. The social worker named Jeri was extremely difficult to communicate with she wouldn’t return calls or emails didn’t seem interested in advocating for my Mom only in getting her discharged as soon as possible. Upon discharge she had me make transportation arrangements? Discharge was scheduled for April 15 l communicated I wouldn’t be present she said no worries I can sign the paperwork over the... read more
Everything was good but the food service. 9 out of 10 times they brought me the wrong meal. Some of what they brought was so horrible that a lot of patients refused to eat it. I had to order from outside the facility a few times to get any food at all. Obviously they don't have a nutritionalist. They need to contract out food service.. The PT and OT was very good.
Staff did not seem to care about their patients. Food was terrible. No proper communication everyone had different answers. No body knew what was going on
Inadequate medical care. Diabetic diet and cpap not provided after hospitalization at Good Samatitan Hospital. After about 2 weeks at Avalon, I was hospitalized at St. Francis Hospital for chest pressure caused by edema, needed IV lasix to loose 20 lbs of fluid. Communication lacking between areas of facility. Podiatry not provided for October or November even though I was "on the list". Blood tests requested by POA revealed liver damage, anemia- vitamin B12 deficient, vitamin D deficient, malnutrition, hypothyroidism, kidne... read more
All the staff that worked on my floor were great. From housekeeping to nurses. My issues only came with social services and administrator with the urgency that was shown to have me leave. My insurance company said they would not pay, so out you go! No one tried to help me stay longer. Yes I could appeal, but if I lost the appeal, I would have to.pay 600 to 800.00 a day. Now that I'm home, I have had 2 close calls of falling. I'm not strong enough to be home . The home health people are going to call me in a week. So for a w... read more
Have to wait too long for someone to come once you ring the buzzer Fell out of bed 3 times. 1st time took someone 29 minutes to come help me. 3rd time I wax told that I doing it to get attention. Most if the staff are rude or can't speak English very well
I brought in clothing for my uncle to change into with direction for them to change him. 2 weeks later I asked about the clothing because he was still in the same pair for clothes he was sent in with. They never changed him but instead put the clothing away in draws. And also he needed an appointment at the hospital and they were supposed to call and make an appointment according their availability but an appointment was never made. I request for things to happen and they say yes upfront but doesn’t make it happen. Very frus... read more
Communication was lacking and often when communicating it was as if was a burden and was told that I was taking them away from other patients. In the time of Covid when I can not be there with my mother, I expect to be able to speak to a caregiver from the facility on a daily basis and not just once a week, especially in light of the ongoing medical issue she was having. They also took her boot off her foot for over 24hrs. This boot was in fact her acting cast for her fractured ankle and foot. Prescribed a medication that i... read more
First of all.. Your food is HORRIBLE. Need to hire people who can cook. The nurses, not all of them, don't know what they're doing. They give mixed information...better yet, they lie. Was told I have an f appointment with the surgeon that operated on my ankle and that was a lie. Kept telling me throughout my stay that I had an appointment coming up but butwhen I called the doctors office was told there was no future appointment at all. My for was suppose to be put in a boot and want till a week later after I called the offi... read more
