An amazingly lot of paperwork to complete. Some questions didn't even seem to relate to my medical situation. And some of the questions were impossible to complete because there was a very limited space to write a response, i.e.my hobbies, etc. And there was excessively large noise in the waiting area, so I moved to the far end of the area to complete the forms. For my next appointment I plan to take the paperwork home and complete them prior to the next appointment.

It was impossible to get an appointment by phone. I spent a lot of time trying to get an appointment. So, I went to the building, and they got me an appointment in just a few minutes.
