We were referred to Jolene Nelson who was extremely compassionate, professional and informative. My dear wife has just completed a 2 month stay. She went in on a guerney for rehab to regain her strength, and walked out on her own power. During her stay, she had several problems to address - the most acute of all her throwing up everything she ate or drank. While, she had outpatient appointments at a Urologist, and Oncologist. My dad stayed at this facility twice. We had other choices. After his second hospitalization we would never have chosen any other facility. I have been a nurse for almost 30 years, a manager for 10 and to say I am picky and hard to please would be an understatement. I found the nursing staff to be competent, caring and well trained. A mistake I will never forgive myself for .... She arrived there May 23 and by June 15 I no longer had my mother . The absolute worst I've ever seen in my entire life. I do home care for for a living and could never treat a human the way they do . Most of the staff deserve to rot alone one day . I wish I could give this facility zero stars. The care my grandmother received here was deeply concerning and ultimately devastating for our family. She was only a resident for 12 days, and during that time my mother visited multiple times every single day--morning, afternoon, and often again in the evening. What she witnessed was alarming. Mom wasn't there long enough but the little time we were there we could see the care she needed would not be sufficient. We had to make the decision to get her back to the hospital. It's 24 hours to see the Nurse Practitioner, needed Iv fluids had to order 4-5 hours waiting , blood draw 2 hours as for the place it smelled clean enough but could use a makeover. I'd give them zero if possible. We placed my father, who has Alzheimer's disease, at Apache Junction Health Center for a 30-day respite stay while my mother and I recovered from surgeries in January 2026. Unfortunately, our experience was deeply concerning. A formal complaint has been made with ADHS and is under investigation. This place is the worst. A week after my father fell out of bed ( yes they knew he was a fall risk, and yet they did not put rails on his bed) he passed. Nothing but negligent uncaring doctors and nurses
Care is not great, I've heard of places that are worse. My mother in law was there for just under 2 weeks. They put her in a diaper so they didn't have to help her to the bathroom. The last 2 days she was soaked when we got there. She did her therapy so she could come home after a short hospital stay. If you're going to have someone there, go check in a few times a day.
