Don't bring anything of value to decorate their room because items do get passed around by the other residents. Make sure your female loved one is okay with having a male caregiver because Charter allows men to clean and change female residents. Label everything.

The TV room, furniture and carpet where the Residents live smell of urine. I would spray the furniture with Lysol and the carpet with Febreze. They never shampoo or vacuum their carpet, nor do they clean their furniture. Very short staffed. The Residents are not toileted, as I witnessed them and my mother soaked and smelling of urine. I washed my mother's laundry but her clothes kept coming up missing although her closet was locked. I took my complaints to management and they were never addressed! The reason for our ratings is because not everything has been what we were told during the initial tour. They have a high turnover of staffing, and there is no push to include activities for the residents.

Citidel used to be as wonderful an experience as a senior with mental and physical challenges could expect. My dad has been there a couple of years now. When we came, there was a manager named [name removed] who was excellent and very caring. Since then, management has changed and the staff is significantly reduced. There are seniors walking in and out of random rooms, falling because there is limited help to ensure their safety and my dad has even worsened due to the staff not ensuring that he remembers to drink his water r... read more

My firsthand experience spans almost 5 years. When my mom was initially placed here it was Arden Courts, which was a very good owner, within a two year period, overnight, the facility was no longer Arden Courts and became Citadel Estates of Hazel Crest. There was no announcement that the business had changed hands, or any information provided about the new owners. There have been four different executive directors in less than five years. My dad lived at Citadel of Hazel Crest for 3 months and concerns began less than a week later; from an aggressive dog, uncleanliness of facility and dad, and improper behavior of caregivers, the final incident is when the director of the facility, English [Removed], stated, "I condone their behavior and sometimes I may forget I'm at work."
