Too many forms to complete, I called a week ago and asked to have the forms emailed and your officer agreed. I never received the emailed forms,

Appointment time was 4 pm (scheduled a month ago) and I didn’t get called in to a room until 5:03 pm. After a long day teaching that is really not respectful of my time!
Dr. Berman was great, however the office way overbooked him with patients! He appeared to be doing his best to see all the patients and still provide quality care. I was at the office for 3 1/2 hours which is absolutely ridiculous. The drive to and from my home to the Ontario office takes at least 1 1/4 hours each direction, so I spent nearly the entire day sitting, which is extremely painful for me. This overbooking of patients is definitely not acceptable.
Omg my appointment was at 2:30 and it now 4:20 I’m pissed off to the max I’m late for work

Very Well, it was a pleasure to be there and work on my shoulder also Cindy was very understandable.
For 1/4/24 at Ontario, .stretching exercises, which showed me what exercises I can do at home, and electro..?... but today, 1/4/24, st 3:15pm appointment, I missed the heating pad. The therapist said she didn’t put it because when sitting on a chair, it gets too hot, but I had it other times when I sat on the chair. So today, the heating pad was not used, around the surrounding area where my surgery was. The heat along with the heat is comforting on my back.
