My reasons for giving Trellis Centennial a five-star rating. Professional, compassion and timely care from the entire team at Trellis Centennial. The daily care provided by the nurses and CNA's was great. Those who helped my wife were truly first class. CNA Carmela was exceptional. The PT and OT staff wanted only the best for her. My 88‑year‑old dad spent 24 days at Trellis Centennial Rehabilitation Facility after a six‑day stay at Centennial Hills Hospital, where he was treated for malnourishment and severe dehydration caused by a brutal case of shingles. Once he was deemed "medically stable," the hospital transferred him here for physical rehabilitation. Outstanding Care and Support from the Staff at Trellis Centennial I cannot say enough good things about the incredible team at Trellis Centennial. You know you have chosen the right place for the care of your loved ones when you are greeted walking in the door by Julie. She makes you feel comfortable to ask any question you may have unanswered. People don't care how much you know but they surely want to know how much you care, Julie is a fine example. What a gem!

My mother was transferred to Trellis back in April of 2024. She had two compression fx of her lower back, along with diabetes, heart disease, mild dementia, and stage three kidney failure. I thought it best to observe the facility for a while, get to know the staff, the physical therapy, and the caseworkers before forming an opinion. The first thing on my pro list was the private rooms. ( the last time she was in a rehab, her room mate was a problem).
