Best thing going for this place is the smallness and one level. They do not monitor what they eat. Residents are given a menu in the morning to pick for tomorrow. Do not have a diabetic or congestive heart failure choices. Management - where to I begin. Just do not do it - go elsewhere. They want your business but not the task of educating the resident and family members how things work. Noone is really sure what the left hand or right hand is doing. On my moms move in day she had to be there at 1pm - not sure why. I thought... read more
I wouldn't drop my garbage off here for them to take care of! Everyone was incredible and then they took our money! Fed my diabetic father cupcakes. Found mounds of soiled clothes hidden in his closet and when confronting the staff, suddenly, things got worse. He was there for 3 weeks, they took [amount removed] from us and are now suing us for another[amount removed]. There will be more to come... I gave them weeks to return the money and the director who was previously a CULINARY director was let go (no surprise) They clai... read more
This community has awful care givers. They speak to Alz and Dementia patients with such anger. My dads closet was full of used depends and soiled clothing. His sugar levels were emergency levels! This even being a diabetic as well. My dads health quickly declined here. He didnt last long at all. This community also owes we over 3k in refunds and they refuse to return calls. Leadership refuses to return calls when your express concern. They will ship your loved ones out in an ambulance vs giving them the care they need in the... read more
The new property Director is terrific (he has his work cut out for him). The Sales Manager will be your friend right up until you sign the dotted line then no more return calls. Took my mom to see if there was interest in the community. We decided not at the time (I was working with her drs. because of her worry issues). I let the lady filling in for the sales person know that I was her POA. We left and I took her home. Another resident called her and put the guilt trip. So she went to the office with her and had my mom sign... read more
Walls and doors were scuffed and dirty. Didn't seem as clean as we would want

My family member recently moved into this facility. A lot of promises were made on the front end prior to the move. She has a Rx for a very special medication that is critical to her health, that has to be delivered by Fed Ex each month & signed for. Director of Nursing, [name removed] was aware of this from day one. The Pharmacy that sends this Rx contacted me, as they do each month to verify delivery. I did not have the address with me, as I was driving, but gave them [name removed], DON, phone number to verify address. I ... read more
We placed my Dad at Provision at Hermitage last September. They were friendly, his apartment was kept clean, and he was close to my brother. However within a few weeks its was obvious that the personal care staff was not helping to shower him, reminding him to brush his teeth, put on pajamas , or putting out clean clothes for the next day. They said it is Tennessee law that they cant force a client to do anything. Dad had been at a previous home for 5 years and had been willing to do whatever he was told. But he was demented... read more
