My loved one passed away at Santa Cruz Post Acute, and our experience there was heartbreaking. She was transferred from a Monterey hospital after femur surgery to receive physical therapy and become strong enough to return home, but instead her condition declined under their care. During her stay, she lost a considerable amount of weight, and although her surgical wound had healed, she developed bed sores on the bottoms of her feet and lower back that were not properly communicated to us (the family.) BEWARE OF POORLY RUN SKILLED NURSING FACILITY My brother was placed here for rehab after being in the acute care hospital. He is mentally and physically impaired and unable to speak up for himself. This facility provided subpar, sloppy care including medication errors, not providing enough showers or hygiene, not enough nutrition...no snacks were provided between meals, and he was always hungry.. Had a family member stay here for recovery after being discharged and I would not recommend anyone to take their loved ones here. The quality of care is non existent, along with hygiene standards. Upon arriving on the first day, our family member had to be put on life support. In the middle of the night, the machine broke and she was left unattended after repeatedly hitting the call button for 3 hours. She thought she was going to die because no one was coming to help. The moment I stepped into this place the whole vibe was off. Don't expect a greeting from the front desk because you won't be getting one. What you can expect to see is patients up and down the hallways and the smell was horrific. My grandmother stayed here for 1 week before we were luckily able to move her. She was not changed regularly, or transferred properly. She was put on the facility floor with no socks on which doesn't seem like a big deal but in healthcare it is. My special Needs brother was in this facility. There were constantly problems with medication dispensing concerns. Labs were not being done per doctor's orders. Phone in room was out of service for days and multiple requests to fix it were made. Different patients would enter into his personal space and try to take his things. I had to have his personal caregiver sit daily with him as I could not for personal reasons. Food was terrible. Quality of care was not good.

I have never submitted a negative review before because I believe in treating others as I wish to be treated. However, I feel it is necessary to share my observations, even though I am not a permanent staff member. Feedback from both nursing staff and patients indicates significant dissatisfaction with the quality of patient care, largely due to chronic understaffing, especially on weekends. My mother was here last year. At first it appeared all seemed well. When I would try and call the staff, it became a nightmare. They were clearly short handed of CNAs and Nurses. Meds were NOT given in time and this became a very big issue. I live 3 hours away so I could not get there quickly. One night my Mom called and said it was time for her to die. I've had a family member stay here a few years back when Rusty, the administrator, was still there. It was great... I recently went back to the facility because my father is staying there due to an accident he had. I've been trying to get a hold of social services for over a week now, and I haven't heard anything back. The meal portions are super small. My toddler won't even get full with that... Mom is year 6 in care facility , last 1 year at Santa Cruz post acute care . Rec staff left her in her wheel chair next to her bed unattended, she tried to get up and fell, nurse found her down bleeding, 17 stitches in her forehead and me five hours w her in the ER.
