My dad was brought here after a fall. It was a terrible experience for him and those visiting him. Listening to our patients constantly begging for help but aides seemed to never be around to help. The same visit we experienced a patient trying to leave facility. Place is not cleaned regularly as I had put a piece of paper in the lobby table as a test and it was as still there as he next day. As a family we decided that it was best for my father to o come home. We discovered the medication that he was discharged with was... read more

Are you kidding me for 5 stars? My father lasted only one night there. The place is filthy, smells so bad, the staff are lost and have no clue what they are doing. When we asked them a question they start talking with their own language to each other in front of us and handed us his chart to look for a phone number. My sister had to fix the mucus suction unit because the nurse didn't know how and told us this is the only one that they have! 5 stars for what? This place doesn't even deserve a 1 star.

Be sure to visit several facilities before you decide where you would like your loved one to reside. This facility is not bad. They have a 4 star rating. We felt the atmosphere was not conducive to our mothers recovery. Staff was really nice and sensitive to needs.

Business response:

Hi Ann, Thank you for your review. Though I'm disappointed to hear that we weren't a good fit for your mother, I do appreciate your feedback. And, I'd love to learn more about your concerns with the atmosphere so that we can improve. Would you mind contacting me at (310) 221-1827? Thank you, Daniel, Administrator

It is hard for me to share an issue that is a cruel practice that shows how easy we throw someone to the streets. When they decide they want to keep the only money they get minus $30 The facilty so kindly gives one a choice of turning over their money making it difficult to survive here with a potter's income. Recently three people, one woman and two men chose to head back to the streets. I probably will be next. I realize we get our basic needs, but I like most have monthly bills to pay. For Sunnyvale Post Acute Care (Plum)... read more

I wish I could report better, but here's my story. My dad had been a resident here for 4 years. Nothing special. He was fed okay, his requests were addressed okay. He was not, by nature a complainer so I think he was well liked by the center, maybe BECAUSE of his gentle disposition. But then this year: On December 1, 2016, my dad calls 911(yes, MY DAD) because he was feeling a "little lightheaded". My dad, not being much of a complainer, had to have felt pretty cruddy to be dialing 911. Well, they picked him up and ... read more

Now Let's just say my grandfather's stay here was by far the worst experience he and our family has ever had! "Nurses and "cna's are running around like chickens with their heads cut off. They claim to bathe patients twice a week.?! He was only bathed once during his stay. Personal hygiene is where they lacked my grandfather had rotted up food In his mouth from not being able to swallow which once again should have be taken care of and the responsibilities of the cna's that work there. My grandfather needed assistance being ... read more
