Sunnyvale Post Acute Center is a good place to stay. My experience with CNA are excellent and staff are great. Therapy and staff are excellent. All managers are all willing help without any hesitation and really have a great team to take care of all patients or residents. I will highly recommend who need care to this facility.

Do not send your loved ones here! Their negligence caused my grandmother's death. If I could give zero stars, I would. My grandmother was admitted to Sunnyvale Post Acute Center after being discharged from the hospital. She was an elderly woman with a history of stroke (right-side weakness), atrial fibrillation, diabetes, and kidney failure. When she arrived, her kidney function was already fragile at 14%. This place really don't care about patients. We understand that there are shortage of staff, but we can tell the difference of not caring and not able to care. There were many issues during my dad's stay. One thing particular disturbing was that they kept trying to make him to stay longer. We first requested on Saturday. The nursing supervisor said they can't do normal discharge on weekend. They'll discharge him on Monday or Tuesday the latest. Update since April 8th since we took her out of the facility we had an appointment with her primary doctor as well as the pharmacist and they were very surprised why the Dr. At the Sunnyvale post acute facility would prescribe medication for her, but not actually have the medication sent out. When she was discharged they sent 3-4 medications for me to pick up when on their "list" she was supposed to be taking 8 medications in total. First off we gave this place a second chance now with my dad's recovery/PT, first was with my mom. With my mom it was long periods of time for help with the use of the restroom and finding out when we got home missing clothes. Now recently with my dad I was visiting and they were going to give him double dosage of meds luckily my brother was still around when I was visiting and questioned it to the nurse who just started her shift there was no communication between the two nurses about the medication being given!!!! I had to walk out during the tour. The smell of urine was overwhelming from the entrance of the facility. If the building is in such disrepair and the place smells so horrible, how must they care for the poor patients in this facility. I was appalled!!

This place is no good do not send your person there that do not pay any attention to their people that are there to get care I had a horrible experience they did not take care of my husband do not send your people there

My aunt had to come here for some rehab after an illness. First of all, the staff don't follow any kind of consistent COVID 18 safety protocols. Some staff wore no masks, some wore them incorrectly, some in n95 masks, some just surgical, som with face shields. Every visitor has to wear the mask the facility requires and hand you, plus a face shield and have your temp taken. Every time. Why don't the staff? Sunnyvale Post-Acute Center I wish you have a better management. This place does not work responsibly. Family members of patient bring food in for patient at Sunnyvale Post-Acute center , the food was mixed up for another patient.
