Get directionsDo not send your loved one here. My father was at Encompass for about 10-15 days and while at the facility he fell 3 separate times IN THEIR CARE. The first time he fell out of his bed because they did not put the bed rails up, the 2nd time was out of his wheelchair because they did not strap him in and he fell out right onto his head where he now has a massive scab, and the 3rd time they allowed him to try and get into his wheelchair on his own and he fell again. Amongst many things there was inconsistencies with staff f... read more

THE WORST. My dad had a stroke. We moved him to this facility after he was already at a skilled nursing facility that rehab'd him back to walking and standing. We chose to move him here because acute rehab was supposed to be offered. There was NOTHING acute about this place. In fact, he regressed. He was walking, transferring, getting stronger at the previous facility. For a week they did almost nothing with him so they they had to go back to using lifting equipment to get him in and out of his chair and bed. He had progres... read more

My dad passed away last May not long after returning home from Health South. He was released from Health South after losing 18 pounds at the facility, had a urinary tract infection and pneumonia. He was sent to the emergency room within hours of returning home and never recovered. Being out of state, I depended on the case manager, Lisa Schwimmer, to keep on top of the care he was receiving. We were so unhappy the response we were receiving, that we met with Lisa's supervisor, Natasha Buhrman-Kleier, with not much of a r... read more

It appears that I cannot rate for under 1 star. My father, who of course is precious to our family, was a patient at HealthSouth in Mesa. He had a stroke on Friday 3/28/15. He arrived at Health South in Mesa for rehab Thursday 4/3/15 and was there until Sunday 4/5/15. We made sure that someone was there with him 24/7. Nurses were not quick to respond to our requests and at times were downright rude. Sunday night my father had a seizure or a stroke - diagnoses is not yet complete. My daughter was with him at the time and... read more

Facility is newer and kept clean. Nurses and CNA's were exceptional but can't say the same about some of the physical therapist's. I was trying to get out of bed and the CNA was trying to help me. The therapist wouldn't let her help me- said she needs to do it on her own. The thing about the bed is when you raise yourself up it dips in the middle and you are trying pull yourself up and I had very little strength. I was about 4 days out having been in a coma for 8 days and I had a 4 inch wide about 1 inch deep wound in th... read more
