This place has illegally used my name and information along with a separate tax id# for illicit purposes. I believe they are using it for their business and implicating me in their activities without my permission as was all on an appointment sheet provided by my doctor ************************ from ************** clinic N92.1/21231 Please advise on how to proceed with an official complaint with police and legal local authorities.Thank you *****************************

First, I called Simon Med to see if they allow walk-ins and was told yes! So my wife and I thought we would walk in and get a x-ray. WRONG ! They don't allow walk-ins! I was told I need a order from my Doctor. I called my doctor and gave them the fax number. After 2 hours I was told they haven't been faxed a order. I thought maybe my Doctor was busy, And we left and decided to call in later. I wish I could give zero stars. My significant other sustained an injury that required a STAT MRI. I was hung up on over the phone multiple times, 3 appointments canceled with no notice, wrong info submitted to insurance for authorization and then having to self ******** to get the treatment he needed. So many people made mistakes during this process and if it werent for pushing back he never would have received the treatment. Went for an MRI at Simonmed on *********** ********. To start with its Not the most pleasant experience. After the traumatic disorienting 45 minute experience was told by the tech named ****** that I could retrieve my clothing at the change room. No offer to undo the backward facing gown I was wearing or to take me to the change room. I went to the Park Blvd. location yesterday. Dont expect to get anything done in a hurry. They will check you in, supposedly, and then forget about you for an hour and a half, all while you wait for a five minute CT scan. Not only that, they said my co-pay for said CT scan was a very unreasonable $125.

There is no rating lower than ONE STAR so SimonMed gets a bonus for this review. SimonMed recently acquired Gateway Imaging in *************, where I have been a patient/customer for years. I arrived a bit early for my Noon appointment for a MRI and was left sitting in the waiting room until a few minutes after 1:00. Led to a changing room, I was instructed to change to a gown and then left there for around 20 minutes. You cannot get an xray scheduled. They will only do walk-ins for xrays. But that typically means you will have to wait way too long (two hours or more based on my experience) to finally get in. Happened to me again today. I expressed that concern at the front desk, and the gal said not to worry, she would get me in. I promptly received two texts saying I was scheduled...blah, blah. I've had imaging several times at Simon Med. They require payment in advance based on what they think insurance wiill require, if they can't reach insurance they come up with a number somehow. The first problem was a $1200 MRI bill that should have been a $50 co-pay. I had to pay $1200 to get the imaging, and then it took me over 5 mos and more than 7 phone calls to get a refund.
