Get directionsBeautiful facility but.... two days after my sisters arrival they found my 73 year old sister on the floor and called me early that morning. They did not know how long she had been laying there so they said. Seriously someone who is gravely disabled mentally , had hip surgery, and no bed alarm. They reassured me that she was ok but how was I to know, they won't let anyone in to visit. Now the other matter is the lack of communication between Trellis and any other facility your loved one may be going. I understand it is the H... read more
This facility may be ideal for some residents with zero complications and a fat income source, but it's a nightmare for anyone who isn't a profiting commodity. Its only redeeming factor is its professional and caring nurses, at least on the surface. The physicians don't do their due diligence or even stick around long enough to know the patient's diagnosis or history. The patients' rights advocates and especially the social worker are unprofessional, illiterate in the most rudimentary medicine/science/logic, and counterprodu... read more

N. K., Thank you for your detailed feedback regarding your mother's stay and discharge. While I can't discuss specifics online due to HIPAA law, I would love to chat offline to better address the situation. Please give me a call at (909) 465-0246 when you have a moment to talk. Gary, Administrator
Still being able to stand on her own, my mother was admitted here a month ago. During those three weeks that she was there, the only people that actually gave 2 cents about her were her CNAs. The doctors here are HORRIBLE. They went in once to introduce themselves and she never saw them again during those weeks. My mom has been in and out of the hospital for 6 months. Due to a rare disease that only her FAMILY was able to to detect on TIME because we decided to do an AMA for her. My mom needed tons of tests and exams but thi... read more
Initially when I went to the facility three months ago for a tour I was pleased as to how nice the staff seemed and how beautiful the facility looked which is why I was ok with recommending this facility for my mom. Unfortunately once she arrived at the facility her health rapidly declined. She developed sepsis, a blood infection and developed pneumonia. She had a seizure in the early morning and instead of taking her to the hospital right away they waited a few hours and then called me to let me know what happened. I advis... read more
First of all I want to thank the nursing staff and CNA's they were wonderful during my mothers Stay. Tonya & Austin were among my mom's favorites while she was there. Here are my concerns and I hope they will address these and make the experience better for future patients coming in. 1. Remember you are your loved ones advocate, speak up when you see something. My mother contracted sepsis while there at the facility. We had to be the one to push for her to be transported to the hospital. I think that this should have ... read more
I would not recommend this place to anyone personally. My grandmother's health declined tremendously just being there one month. I honestly would like to know how these nurses and respiratory therapists still have jobs there. The food does not look appetizing at all. They say that the patients can put in special requests for something else to eat but nothing gets done. The lack of communication between the nurses is mind boggling. We were told my grandmother would be put on a list to get physical therapy, and it never happen... read more
The facility is nice and the staff are very nice but the department heads are rude. Not only to patients family but to their own staff. Especially the DON! I overheard her demeaning a staff member over disinfectant wipes! Just awful!! But overall the facility is beautiful my mother-in-law was treated well
